Plan Services
- Exams: 2 per benefit year
- Cleanings: 2 cleanings per benefit year (in absence of periodontal disease)
- X-rays: 2 sets of necessary x-rays per benefit period
- 15% off General Services (fillings, crowns, root canals, etc.) when paying check/cash
- 12% off General Services (fillings, crowns, root canals, etc.) when paying credit/debit
- 10% off Implant Services when paying check/cash

- Primary Individual: $444
- Secondary Members: $388 (spouse and/or children living in the household until the age of 23)
